That's the yellow button at the bottom of the form - please check your email address as the completed form is automatically forwarded to a Vet, our Sales Team and back to you as confirmation of submission.
The Vet Will Reach Out...
After receiving the completed submission form, the Vet will communicate with us and with you to determine the best course of action. You may be requested to bring a sample of dead chicks to the Vet's office for analysis if he is unable to come to your farm.
After Vet Analysis...
A veterinary report, including any lab results that might have been additionally requested, will be emailed both to you and to our Sales Team. We can assist in explaining the Vet's findings, we can also request that the vet explain them to you personally if you prefer. Please see our terms of sale for an explanation on refunds and replacement chicks.
